Need a Diet Plan

I am 15½ years old and I weigh over 200 lbs. I'm not sure how much exacty, because I don't weigh myself (remind me to get a scale). I do know I'm over 220lbs.
I would like to lose 100 lbs

2 months ago I stopped drinking soda, juice, anything besides water. So I've been drinking water - nothing else, for two months. It's going well, I have no desire to drink soda or juice. :)

Last year in the winter, I started exercising for about a month and saw no results. So I just gave up.

Now I need a food plan and a work out plan, I'm not very good with thinking up meals, or couting calories, anything like this, so I really need help. I have tons of time on my hands.
I'm a very picky eater, I do not like seafood, very little veggies, and some fruits.

I live in the middle of no where so it's hard to take a walk outside my house unless I want to get lost in the forest or bitten by a snake, no not really but I guess you understand what I mean :P
There is a track about 10 minutes away from here, I'm going to ask my mom to take me to that to walk around.

What should I eat?
What should I do to work out?
How do I count my calories?
How do I keep track of everything?

You will have to tailor your plan to the foods you are willing to eat. It is worth mentioning that people are very habitual and can learn to love anything (and give up foods they appear to be addicted to) in the space of 3-6 weeks.
